Produce Subscription Application

Please reserve my subscription for the 2010 growing season. Enclosed is my $50 non-refundable reservation fee. (due by March 1st). Payment in full is due by March 31, 2010. The total due will be $50 reservation fee plus $20 per week per box. (1 box @ $20 X 20 weeks plus $50 equals $450). You may subscribe to more than one box. Print out our application, fill it out, and mail it Calvert Farm (the address is below).

Terms & Conditions

Calvert Farm agrees to provide a box of farm fresh produce each week for 20 weeks. Variety of produce shall be at the discretion of Calvert Farm. Subscription shall start the week of May 16, 2010 and end the week of September 26 , 2010. Subscriber agrees to pick up produce at the time and place they have noted on their application. If produce is not picked up at designated time subscriber may forfeit their produce. The $20 per unit per week allotment does not carry over to any other week or time.
